RN Case Manager for Home Health - FT Days (Monday-Friday)

Health Sense, now a part of Jordan Health Services, is hiring an RN Case Manager.  The hours are Monday – Friday, 8:00 am - 5:00 pm, with on-call rotation. Job Duties

  • Receives verbal reports from field staff.  Ensures appropriate level of care is being requested by field staff.  Negotiates changes to plan of care when needed to maximize clinical outcomes within the patient’s budget.
  • Reviews OASIS assessments for clinical accuracy, works closely with assessing clinicians to correct/clarify answers prior to approving document to be added to patient record.  Builds, approves, and locks 485 Plan of Care from OASIS Assessments within 5-7 days from assessment date.  Ensures post-admit OASIS assessments occur at the right time points and by the appropriate staff.  Completes transfer or desk discharge OASIS when needed.
  • Coordinates patient care to assist patients, families, physicians, and field staff to provide quality patient care.  Includes ordering DME, taking verbal orders, reporting lab results and relaying new orders to field staff.  Acts as a resources to staff for inter-disciplinary referrals, non-compliant patients, abnormal vitals, new or changed medications, wounds, disease processes, patient/staff infections, patient falls or other incident report issues, hospitalizations, or hold orders.
  • Reviews clinical documents for compliance with agency policies, procedures, and payor source requirements prior to approving document to be added to patient record.  Returns insufficient documentation to field staff for correction.  Ensures documentation evidences that services are provided as ordered in the Plan of Care and meets payor source coverage criteria.
  • Ensures patient discharges are appropriate (i.e., all goals were met, no longer meets coverage criteria, etc.).  Receives and approves discharge requests from field staff.  When inappropriate, coordinates with care team to recert patient.  When appropriate, schedule discharge visit, coordinates with remaining staff for adequate documentation is completed (i.e., HHABN, NOMNC, OASIS DC, etc.).
  • Back up other clinical team positions as needed (i.e., Intake, Field Nurses, Scheduling, etc.).
  • Maintain adequate knowledge of Medicare and payor source standards to ensure reimbursement for all services rendered. 

Education & Experience

  • Graduate of an accredited School of Nursing and current Texas State license as a RN in good standing.
  • Minimum of 2 years experience as a Home Health RN.
  • Nursing skills as defined as generally accepted standard of practice.
  • Current CPR certification, Texas Driver License and auto liability insurance.
  • Strong knowledge of applicable rules and regulations, and payor source coverage criteria.
  • Cooperative attitude with the ability to work well with others in a tactful and diplomatic manner to provide excellent customer service.
  • Ability to accomplish tasks in the most timely and efficient manner.
  • Strong written and verbal communication and interpersonal skills.
  • Solid multi-tasking, problem solving and organizational skills.
